In this issue of the Breast Cancer Research and Treatment, there is the encouraging report from van den Hurk et al. [1] that the 5 year overall survival rate of women diagnosed with Stage I–III breast cancer is improving relative to earlier decades. However, there was also apparent discouraging results reported by the authors that there was a parallel unfavorable change in the pattern of metastases over time, with no improvement in survival in the later relapsed metastatic cohorts. Thus, how much of the battle with breast cancer are we really winning.
