Description
In the last few decades there has been a shift in the paradigm of rectal cancer treatment. Surgery, historically the only weapon against rectal malignances, has found valuable allies in other treatment modalities, extending the range of cure to previously untreatable stages of disease. Perioperative protocols have led to a dramatic improvement in local control of locally advanced disease. The integration of targeted therapies with more effective cytotoxic protocols and refined surgical strategies has expanded the treatment of metastatic disease, resulting in improved survival. Advances in risk stratification will allow us to identify patients who may benefit from conservative surgical approaches in the future. This manuscript reviews the accepted standard of care protocols and highlights areas currently under debate.
