Zygomycosis is an emerging fungal disease that is associated with considerable excess morbidity and mortality. Standard approaches to treatment include reversal of the underlying immunodeficiency or metabolic disorder, surgical interventions, and antifungal therapy with amphotericin B lipid complex or liposomal amphotericin B. Among the newer triazoles, posaconazole possesses activity in vitro and in experimental animal models of zygomycosis, and two observational case series have demonstrated promising clinical responses in the salvage setting. This article reviews the current body of preclinical and clinical data and assesses the present and potential future role of posaconazole in managing zygomycosis.
