Background This paper introduces a new classification based on the radiographs and fixation stability and suggests treatment options for each type of aseptic nonunion of a femoral shaft fracture after interlocking intramedullary nailing. Methods This study examined 35 patients retrospectively, who had an aseptic nonunion of a femoral shaft fracture that had been treated by interlocking intramedullary nailing between February 2000 to February 2006 and had been followed up for more than one year. Aseptic nonunion was newly classified according to the bone defect, callus formation and fixation stability, and the surgical outcome analyzed according to the bony union rate and compared surgical outcome with relating factors. Our new classification is as follows: type I, a callus(?) or bone defect(-) and type II, callus(-) or bone defect(?); subtypes A and B were the stability of nailing(?) and (-), respectively. Results Variable surgical options, such as dynamization, exchange nailing, plating and bone graft, were selected according to the new classification. All patients achieved radiological union after an average of 19.8 weeks. Conclusions This simple and comprehensive classification was found to be useful for determining the surgical methods for femoral nonunion after nailing and for achieving better surgical outcomes.
