How well are the urinary symptoms of cauda equina syndrome (CES) recognised by multidisciplinary health care professionals?

severe compression of the cauda equina. CES presents as either incomplete cauda equina syndrome (CESI) or cauda equina syndrome with retention (CESR). In CESI, neurogenic control of the bladder is maintained, but in CESR, this neurogenic control is lost. Consequently, the only differentiation is in urinary symptoms. Aim The primary aim of this study was to assess whether doctors and physiotherapists could identify the urinary symptoms of CESI and CESR when given a list of urinary symptoms. Our secondary aim was to identify in what time period these professionals would offer surgical intervention for CESI and CESR. Materials and method We analysed 44 results. The CES urinary symptoms were ranked on average higher than the non-related symptoms. Results Physiotherapists were better at differentiation of the urinary symptoms. From our questionnaire, both groups wanted to treat CESR within 48 h. CESI was opted to be decompressed later. Our literature review suggests that both types of CES should be treated within 48 h. There is no evidence to suggest that CESI should be treated sooner, but logic dictates that it should be treated before it develops into CESR. Conclusions In conclusion, there needs to be more education on CES, especially CESI. To prevent any patients being affected by this knowledge gap, a pro forma could be designed and distributed to all points of contact for patients with back pain.
