Gene expression profiles characterizing the progression of heart failure in patients with aortic valve stenosis

Description

Background: Preoperative ejection fraction (EF) is predictive for long-term survival in patients with aortic valve stenosis (AS). To gain insight into genes involved in the progression from regular to low EF, we used microarray technology. Methods: Myocardial left ventricular biopsies from male patients with isolated AS and pronounced left ventricular hypertrophy (LVH) undergoing aortic valve replacement were harvested from hearts either with normal EF (>50%) or with low EF (<30%). Total RNA samples were isolated from biopsies and analyzed using Affymetrix HGU133A and U133B GeneChip sets. Results: Data analysis revealed that, while massive deregulation of the transcriptome is associated with LVH, an unexpectedly low fraction of genes is affected by differential expression during the progression from compensated to de-compensated heart failure. Conclusions: Select myocardial gene transcripts, such as fibrillin 1 or versican, might represent promising candidates for future characterization of the low EF state in LVH.
